But what had he contributed? What had been the point of it all?Lying alone in the stillness of the wee hours, he remembered that what he’d really wanted to do was become a teacher.“So what do you do with that, when you realize a life has been spent in a way that wasn’t the best?” asked Jim Rawlings, who retired recently as head of Duke University Hospital’s pastoral services staff.“Most people live in such denial that anything can happen to them. It’s one of the times they confront their mortality. And there’s anxiety often,” says Rawlings, who spent more than 30 years as a hospital chaplain.A common perception of clergy who work in hospitals is that they are there to perform last rites for the terminally ill, or prompt 11th-hour conversions. But chaplaincy has little do to with religion per se. “We try to hear their story,” says Rawlings. “To be able to tell your story is a way to become known and thus become less lonely, and connect with the spirit inside of us.”While a large hospital will try to provide chaplains who match religious patients’ specific faiths — and possibly a humanist chaplain to cover those who are nonreligious — a chaplain of any faith should be able to minister to any patient.“We don’t proselytize, and we’re not looking to change people’s belief systems,” says Amy Strano, director of spiritual care at Mount Sinai Hospital in New York.“We sit with people in the heartbreak of life. It’s about being with them and letting them grieve and share what’s on their hearts,” she explains. “We sometimes talk about the chaplain as a spiritual care specialist” — and that’s relevant whether “spiritual” refers to God, or to secular ideas of connection or significance.As our medical system grows more fast-paced, impersonal and complex, the hospital or health care chaplain’s role has been growing. Many patients need help deciphering the best next steps. Today there are more than 10,000 health care chaplains in the United States, most with master’s degrees, rigorous hands-on training and board certification. Almost two-thirds of the country’s hospitals make chaplains available.With American society becoming increasingly disconnected and less religious, chaplains’ role has become particularly important. “A lot of people (in the U.S.) don’t have community. We don’t live in neighborhoods our whole lives where people know us; we don’t have those places of worship where we’re known. Nobody brings a casserole” when sickness descends, says Strano. As a result, “when there’s a crisis, there isn’t that community to turn to; there isn’t someone who’s there as that counselor.” And there haven’t necessarily been those discussions about the point of life or what might happen after death.Despite their growing numbers, chaplains have less time to spend with patients. Thirty years ago, hospital stays were longer and chaplains could visit with nearly everyone who came through. These days, they’re encouraged to focus largely on the sickest patients.The biggest change, however, is the shift seen across all health care professions, from volume-based to value-based care. In the past 10 to 15 years, medical professionals have stopped simply billing for services rendered; they have to show that those services led to positive outcomes.Chaplains, too, are increasingly being rated by a wide range of measures: better outcomes, lower costs and higher patient satisfaction.Ultimately, “those who don’t demonstrate value will not be funded,” says George Handzo, director of health services research and quality at the Healthcare Chaplaincy Network, one of the field’s big organizations. Hospital chaplains’ services aren’t covered by insurance. So while they’re very cheap by health care standards, a penny-pinching hospital administrator might very well zero them out if she doesn’t understand what they contribute.The Rev. George Handzo. Video screenshotThe problem is, hard data illustrating the results of chaplains’ visits is difficult to come by. “We’ve never been focused on outcomes,” explains Handzo. “We’ve been largely anti-outcomes. ‘We don’t have an agenda,’ you’ll hear chaplains say.” Chaplains have prided themselves on meeting patients where they are, rather than pushing for anything specific.Handzo recalls a visit to a hospital administrator a few years ago who wanted to talk about chaplains’ contributions. “So I went in with all my numbers about cost per visit over thousands of visits,” remembers Handzo. “And she said something like, ‘Oh George, these numbers are really impressive, but tell me: What difference has this made to my patients?’ And I had no idea how to demonstrate that.”Discussions with chaplains may very well result in families declining complicated and expensive interventions for loved ones near the end of life. A sick person who feels at peace might heal more quickly. And patients and their families who get help in talking about their concerns will likely feel more satisfied with their overall hospital experience. These are all critical indicators in today’s health care environment.But there’s been very little monitoring of those results. Chaplains themselves have simply taken it on faith that their efforts benefit patients.Which isn’t to say there’s no data out there. A 2013 study by Harvard researchers illustrated that patients whose spiritual needs have been met have chosen less aggressive care. A 2015 Mount Sinai paper showed that chaplain visits boost patient satisfaction.George Fitchett, a professor at Rush University, is one of the only chaplains with a Ph.D. who is conducting research on chaplains’ effectiveness.“Our health care colleagues have no training to help them understand who we are and what we do,” says Fitchett. “In an environment where the lingua franca is research, we need to be able to demonstrate what we contribute.”That begins, he says, with careful charting and evaluations by chaplains.Trace Haythorn. Photo by Tavits PhotographySome chaplains are pushing back on the move toward data-driven standardization. “It feels like it cuts to the very core of the practice,” says Trace Haythorn, executive director of ACPE, the main organization for accrediting spiritual care education programs. Chaplains worry that maintaining a compassionate, nonjudgmental presence will be clinicalized into a set of prescribed behaviors.Another, more welcome change is that chaplains, as they are integrated into care teams, find themselves spending more time supporting the hospital staff, helping nurses and doctors who struggle with stress and burnout.As practitioners see how chaplains work, too, some are becoming interested in employing chaplains’ techniques. “Doctors are saying, ‘I want to listen deeply to the patient, too,’” says Christina Puchalski, director of George Washington University’s Institute for Spirituality and Health in Washington, D.C., and a longtime leader in uniting chaplains and practitioners. “Clinicians and nurses are beginning to recognize the importance of things not so deeply measured.”As their work gains in credibility with other health care professionals, chaplaincy may follow the path of palliative care. A decade ago, the notion of managing the symptoms of patients with serious illnesses was considered fringy. Since then, the field has grown hugely, and these days it’s frequently covered by insurance.Some observers believe that the day chaplaincy is covered by insurance isn’t far off. Blue Shield of California already pays for some chaplains in its home-based palliative care programs, and Torrie Fields, the senior manager for advanced illness and palliative care at the company, envisions that coverage continuing to grow.“I see a very clear movement towards chaplains being involved in improving patient outcomes,” particularly in cases of life-threatening illness, says Fields. She imagines chaplains eventually serving as the leaders of multidisciplinary medical teams, all covered by insurance.“I think they’re able to see the big picture,” says Fields, who herself experienced chaplains’ transformative power when she was ill with cancer.Fields doesn’t necessarily think research needs to meet the gold standard of randomized, controlled trials in order to demonstrate chaplains’ value. “Maybe there’s been too much of an emphasis on measuring them, fitting them into a box,” says Fields. “If we put chaplains through that level of rigor, they’ll be limited in doing what actually brings support.”Meanwhile, health care chaplains and their fans are excited to see the field gradually growing into adulthood after a long period of adolescence. Society, they say, needs it.“So much of our public discourse is oppositional, rather than listening deeply for the core of who you are as a person,” says Haythorn. “The very best chaplains help people go to those deep places where very little in this culture is allowed to go safely. The biggest and most important questions in life are often asked in moments like that.”This story was changed to correct the description of ACPE. Using statistical tools in Wireshark for packet analysis Tutorial

When using Wireshark, we have various types of tools, starting from the simple tools for listing end-nodes and conversations, to the more sophisticated tools such as flow and I/O graphs. In this article, we will look at the simple tools in Wireshark that provide us with basic network statistics i.e; who talks to whom over the network, what are the chatty devices, what packet sizes run over the network, and so on. To start statistics tools, start Wireshark, and choose Statistics from the main menu. This article is an excerpt from Network Analysis using Wireshark 2 Cookbook – Second Edition written by Nagendra Kumar Nainar, Yogesh Ramdoss, Yoram Orzach. Using the statistics for capture file properties menu In this recipe, we will learn how to get general information from the data that runs over the network. The capture file properties in Wireshark 2 replaces the summary menu in Wireshark 1. Start Wireshark, click on Statistics. How to do it… From the Statistics menu, choose Capture File Properties: What you will get is the Capture File Properties window (displayed in the following screenshot). As you can see in the following screenshot, we have the following: File: Provides file data, such as filename and path, length, and so on Time: Start time, end time, and duration of capture Capture: Hardware information for the PC that Wireshark is installed on Interfaces: Interface information—the interface registry identifier on the left, if capture filter is turned on, interface type and packet size limit Statistics: General capture statistics, including captured and displayed packets: How it works… This menu simply gives a summary of the filtered data properties and the capture statistics (average packets or bytes per second) when someone wants to learn the capture statistics. Using the statistics for protocol hierarchy menu In this recipe, we will learn how to get protocol hierarchy information of the data that runs over the network. Start Wireshark, click on Statistics. How to do it… From the Statistics menu, choose Protocol Hierarchy: What you will get is data about the protocol distribution in the captured file. You will get the protocol distribution of the captured data. The partial screenshot displayed here depicts the statistics of packets captured on a per-protocol basis: What you will get is the Protocol Hierarchy window: Protocol: The protocol name Percent Packets: The percentage of protocol packets from the total captured packets Packets: The number of protocol packets from the total captured packets Percent Bytes: The percentage of protocol bytes from the total captured packets Bytes: The number of protocol bytes from the total captured packets Bit/s: The bandwidth of this protocol, in relation to the capture time End Packets: The absolute number of packets of this protocol (for the highest protocol in the decode file) End Bytes: The absolute number of bytes of this protocol (for the highest protocol in the decode file) End Bit/s: The bandwidth of this protocol, relative to the capture packets and time (for the highest protocol in the decode file) The end columns counts when the protocol is the last protocol in the packet (that is, when the protocol comes at the end of the frame). These can be TCP packets with no payload (for example, SYN packets) which carry upper layer protocols. That is why you see a zero count for Ethernet, IPv4, and UDP end packets; there are no frames where those protocols are the last protocol in the frame. In this file example, we can see two interesting issues: We can see 1,842 packets of DHCPv6. If IPv6 and DHCPv6 are not required, disable it. We see more than 200,000 checkpoint high availability (CPHA) packets, 74.7% of which are sent over the network we monitored. These are synchronization packets that are sent between two firewalls working in a cluster, updating session tables between the firewalls. Such an amount of packets can severely influence performance. The solution for this problem is to configure a dedicated link between the firewalls so that session tables will not influence the network. How it works… Simply, it calculates statistics over the captured data. Some important things to notice: The percentage always refers to the same layer protocols. For example, in the following screenshot, we see that logical link control has 0.5% of the packets that run over Ethernet, IPv6 has 1.0%, IPv4 has 88.8% of the packets, ARP has 9.6% of the packets and even the old Cisco ISK has 0.1 %—a total of 100 % of the protocols over layer 2 Ethernet. On the other hand, we see that TCP has 75.70% of the data, and inside TCP, only 12.74% of the packets are HTTP, and that is almost it. This is because Wireshark counts only the packets with the HTTP headers. It doesn’t count, for example, the ACK packets, data packets, and so on: Using the statistics for conversations menu In this recipe, we will learn how to get conversation information of the data that runs over the network. Start Wireshark, click on Statistics. How to do it… From the Statistics menu, choose Conversations: The following window will come up: You can choose between layer 2 Ethernet statistics, layer 3 IP statistics, or layer 4 TCP or UDP statistics. You can use this statistics tools for: On layer 2 (Ethernet): To find and isolate broadcast storms On layer 3/layer 4 (TCP/IP): To connect in parallel to the internet router port, and check who is loading the line to the ISP If you see that there is a lot of traffic going out to port 80 (HTTP) on a specific IP address on the internet, you just have to copy the address to your browser and find the website that is most popular with your users. If you don’t get anything, simply go to a standard DNS resolution website (search Google for DNS lookup) and find out what is loading your internet line. For viewing IP addresses as names, you can check the Name resolution checkbox for name resolution (1 in the previous screenshot). For seeing the name resolution, you will first have to enable it by choosing View | Name Resolution | Enable for Network layer. You can also limit the conversations statistics to a display filter by checking the Limit to display filter checkbox (2). In this way, statistics will be presented on all the packets passing the display filter. A new feature in Wireshark version 2 is the graph feature, marked as (5) in the previous screenshot. When you choose a specific line in the TCP conversations statistics and click Graph…, it brings you to the TCP time/sequence (tcptrace) stream graph. To copy table data, click on the Copy button (3). In TCP or UDP, you can mark a specific line, and then click on the Follow Stream… button (4). This will define a display filter that will show you the specific stream of data. As you can see in the following screenshot, you can also right-click a line and choose to prepare or apply a filter, or to colorize a data stream: We also see that, unlike the previous Wireshark version, in which we saw all types of protocols in the upper tabs, here we can choose which protocols to see when only the identified protocols are presented by default. How it works… A network conversation is the traffic between two specific endpoints. For example, an IP conversation is all the traffic between two IP addresses, and TCP conversations present all TCP connections. Using the statistics for endpoints menu In this recipe, we will learn how to get endpoint statistics information of the captured data. Start Wireshark and click on Statistics. How to do it… To view the endpoint statistics, follow these steps: From the Statistics menu, choose Endpoints: The following window will come up: In this window, you will be able to see layer 2, 3, and 4 endpoints, which is Ethernet, IP, and TCP or UDP. From the left-hand side of the window you can see (here is an example for the TCP tab): Endpoint IP address and port number on this host Total packets sent, and bytes received from and to this host Packets to the host (Packets A → B) and bytes to host (Bytes A → B) Packets to the host (Packets B → A) and bytes to host (Bytes B → A) The Latitude and Longitude columns applicable with the GeoIP configured At the bottom of the window we have the following checkboxes: Name resolution: Provide name resolution in cases where it is configured in the name resolution under the view menu. Limit to display filter: To show statistics only for the display filter configured on the main window. Copy: Copy the list values to the clipboard in CSV or YAML format. Map: In cases where GeoIP is configured, shows the geographic information on the geographical map. How it works… Quite simply, it gives statistics on all the endpoints Wireshark has discovered. It can be any situation, such as the following: Few Ethernet (even on) end nodes (that is, MAC addresses), with many IP end nodes (that is, IP addresses)—this will be the case where, for example, we have a router that sends/receives packets from many remote devices. Few IP end nodes with many TCP end nodes—this will be the case for many TCP connections per host. Can be a regular operation of a server with many connections, and it could also be a kind of attack that comes through the network (SYN attack). Microsoft Cloud services DNS outage results in deleting several Microsoft Azure database

first_imgOn January 29, Microsoft Cloud services including Microsoft Azure, Office 365, and Dynamics 365 suffered a major outage. This resulted in customers experiencing intermittent access to Office 365 and also deleting several database records. This comes just after a major outage that prevented Microsoft 365 users from accessing their emails for an entire day in Europe. Users who were already logged into Microsoft services weren’t affected; however, those that were trying to log into new sessions were not able to do so. How did this Microsoft Azure outage happen? According to Microsoft, the preliminary reason behind this outage was a DNS issue with CenturyLink, an external DNS provider. Microsoft Azure’s status page read, “Engineers identified a DNS issue with an external DNS provider”. CenturyLink, in a statement, mentioned that their DNS services experienced disruption due to a software defect, which affected connectivity to a customer’s cloud resources. Along with authentication issues, this outage also caused the deletion of users’ live data stored in Transparent Data Encryption (TDE) databases in Microsoft Azure. TDE databases encrypt information dynamically and decrypt them when customers access it. As the data is stored in encrypted form, it prevents intruders from accessing the database. For encryption, many Azure users store their own encryption keys in Microsoft’s Key Vault encryption key management system. The deletion was triggered by a script that automatically drops TDE database tables when corresponding keys can no longer be accessed in the Key Vault. Microsoft was able to restore the tables from a five-minute snapshot backup. But, those transactions that customers had processed within five minutes of the table drop were expected to raise a support ticket asking for the database copy.
